Reporting notifiable incidents

A person conducting a business or undertaking (PCBU) must notify WorkSafe as soon as they become aware of a workplace fatality, serious injury or illness or dangerous incident.
Serious injuries or illnesses may include those requiring immediate hospital treatment, or cases where a medical practitioner determines the person is unable to perform their normal work duties for at least 10 consecutive days from the date of the injury or illness.
Dangerous incidents may include events such as uncontrolled escapes of substances, fires or explosions, or failures of structures or plant that pose a serious risk.

To report a death, serious injury or illness, or a life-threatening dangerous incident call 1800 678 198 immediately and preserve the site.
 
For any non-life-threatening dangerous incidents, preserve the site and immediately notify WorkSafe -  Notification of Dangerous Incident.
